IT’S TIME FOR CAMP, COLLEGE AND SPORTS PHYSICALS!!

The Cresco and Lime Springs Medical Clinics will once again be offering physical exams as required for summer camp, college entrance, and sports (not college sports) during June, July and August, 2008, by appointment only.

• A parent/guardian needs to either accompany their children or plan to come by the clinic prior to the scheduled appointment to complete the required paperwork.  No physical will be performed without the required paperwork and parent/guardian signature.

• If your insurance pays for an annual physical, we can perform the required physical in conjunction with that exam.  (Please make sure the receptionist is aware of the need for a full physical and the fact that insurance will cover it.)  Otherwise, payment will be expected at the time of service.
  

• No other problems will be addressed during the special athletic, camp or college physical visit.  If additional services are required, you will be asked to schedule a separate appointment, and those services will be submitted to insurance or billed to you at the customary rate.
  

• Athletic physicals after June, July or August will be billed to the patient or your insurer at the customary rate.

Sports/Camp Physical Exam:  $30/June & July; $40/August
College Physical Exam:  $82
